Junior League - Photo Consent Form

We are required to gain parental signed consent for the use of photography within the Swim Training Scheme and City of Leeds Swimming Club events.

If you give consent, your child may be photographed during their involvement whilst representing City of Leeds Swimming Club during our participation in the Junior League 2023. This will be the White Rose round held at John Charles Centre for Sport on Saturday 3^rd June, as well as the Junior League Grand Final on Sunday 26th November.

Photos taken at the event may be used across the City of Leeds Swimming Club Website and Social Media platforms. Please complete the form if you are happy for your child to be photographed and images shared in this way.

Only swimmers who have completed the signed consent form will be allowed to take part in individual and team photographs.
